Provider Appointment Process: Quick & Easy Guide
Before You Start
Have you received your resident state license after becoming a GFI Agent? Complete this first.
Looking for your appointment status with ETHOS? Go here.
Requests: Complete one provider at a time:
Carrier: Select the carrier.
First provider: Ameritas (BROKER) for ETHOS IULs.
Contract: Select Fixed Life & Fixed Annuity.
State: Choose your state (and any other states with pending business).
E&O & Training: Should be already uploaded.
Carrier Questions: Answer any additional ones.
Sign: Apply your signature and click OK.
How to Get Appointed with Most Providers:
Direct Deposit: Double-check it’s set up correctly in your profile.
E&O Certificate: Have this ready to go.
Continuing Education: Make sure you've done:
AML
Annuity Course
Long Term Care Course (if Health Licensed)
Create Your SureLC Account:
Go to SureLC under the GFI link and create an account.
Complete NIPR Verification
3. Your email = your ID. Click CREATE ACCOUNT.
NOTE: If you’ve used SureLC before, your info might be pre-filled. Double-check it!
Contracts: Once appointments are finalized, review them here.
SureLC Fee: There’s an $85 fee for platinum provider appointments, but GFI covers it upfront and deducts it from your commissions.
How to Get Appointed with Corebridge Life
Make sure your appointment request has been submitted in SureLC before you request appointment to be expedited.

How to Get Appointed with Annuity Companies
You can submit appointment request directly in GFI SureLC for Corebridge Annuity, ANICO & Global Atlantic.
If you have business to submit with North American Annuity, Equitrust, SILAC or American Equity you need to complete the google form request and then you will receive an email to apply.
